Movie Credits - Wedding Participant List

Please have your wedding date booked and confirmed prior to filling out this form.  Clients are asked to complete this form 2 to 3 weeks prior to the wedding day.

The main purpose of this form is to provide us with all the names of people who should be credited at the end of the video.  Normally we credit people who make up the wedding party, parents of the wedding couple, people involved in the ceremony, reception, etc...

Please fill out all the fields that pertain to your wedding party.  If you have other people that you would like credited at the end of the video, please use the last text box entitled "Other People". Also keep in mind that the names you provide will displayed within the end credits as they appear within this form.  So please ensure that the spelling is correct.
